Extremely uneven sidewalk. Trip and fall hazard.

The sidewalk in front of 2478 Alexis Road needs repair. The sidewalk dips and when it rains the sidewalk gets partially covered in mud and water. This is a slip and fall hazard, walking my dog at night I slipped and fell on the sidewalk....
